Each disease is listed and assigned a number. Each disease is given its own page. The registers contain no patient names or details. Each disease listed gives the numbers of people diagnosed as suffering from the diseases or illness in each year.

NB: There is no key to the numbering system assigned each disease.

Daily diary of the hospital or ward, registering admissions, operations, acute patients and outpatients.

The top of each page lists the duty doctor for the day.

The diary is set out in an (informal) A4 desk diary, with one day on each page.

Each page contains the following details for each patient:

- Last name

- First name

- Procedure

- Ward

- Time

- Doctor

- Patient/case number

Patient details are sometimes annotated with Acute, later diary has the page broken into: Acute, Arranged, and GOP (General Outpatients).

This seems to be an informal register of perinatal deaths, still births and caesarian section surgeries from Christchurch Women's hospital as the format is a school exercise book. It is possible that a series of formal registers exist recording perinatal deaths, still births and caesarians from Christchurch Women's Hospital.

Perinatal deaths and still births are listed from the front of the book, caesarian sections are listed at the back, and in the middle.

The register contains few details besides the following:

This series consists of two mortuary undertaker signature books from Christchurch Women's Hospital. Books list the deceased and where/whom they were disposed to.

This series consists of what appear to be informal registers of births at Christchurch Women's Hospital.

The registers are contained within school excercise books and list maternity patient details.

The books contain sparse details, such as:

- Hospital/ patient / case number

- Sex of baby

- Occasionally lists if the baby was stillbirth (SB) or neonatal unit (NNU)

This series contains one small softcover notebook listing names and dates of patients and babies for which post-mortem reports were done.

There are few details in the book: name of patient, usually the sex of the baby, date and occasionally a file reference.

The post-mortem reports themselves are not extant.

These hard bound volumes record the following information, one page per week:

These volumes record "outdoor" maternity cases, i.e. home births, attended by St Helen's Hospital staff from 1907 to 1945.

Each printed page is divided into two sections, and records the following information in the first section:

Case number, date name and address of patient

Name of nurses present, and times of arrival, delivery and departure

Notes on birth such as presentation of baby, or whether a doctor was called

The second section records the "Puerperium" (the period after the birth), with space to record ten days of visits from a midwife, showing the time of arrival and departure, and the nurse's name.

Finally, the date of discharge is recorded.

These volumes record "indoor" maternity cases, i.e. in-hospital births, at St Helen's/Christchurch Women's Hospital in Christchurch from 1907 to 1981.

The earlier volumes are printed, and allow two pages per birth. The first page records the following information:

case number, date, and names of nurses present at birth

patient's name, age, address and number of pregnancies

details of the labour such as the time it began, and the time of each stage

other information about the birth, such as the presentation of the baby

information about the baby such as sex, weight, length, temperature and pulse

information about the placenta

The second page for each birth records notes on the case, such as drugs given during birth, post natal treatment and date of discharge. Sometimes left blank.

This volume contains two series: the first 20 pages show indoor case accounts for St Helen's Hospital Wanganui [Whanganui] from 1932 to 1933, and then from page 21 to the end of the volume are St Helen's Hospital Christchurch case accounts from 1944 to 1948.

These registers record patients' operations at St Helen's Hospital, Christchurch from 1969 to 1979.

The double pages are headed: North Canterbury Hospital Board Record of Operations Performed.

All of the plans in this artificial series, excluding one, were created by the Public Works Department in the 1880s, and relate to the Central Block of the Hospital, which was designed by Public Works Department architect John Campbell in c.1883. Construction of the Central Block commenced in 1886, and the building was completed in 1888, adding to the large complex of permanent hospital buildings which had been erected by the government on the Sunnyside site since the early 1870s. Additional Central Block Contract drawings by Campbell can be found in our Wellington Office's Public Works Department plans series.

The other plan, entitled 'Nurses Quarters Receiving Block', looks to be a c.1870 Benjamin Mountfort original. Mountfort created the first plans for the permanent buildings to be erected at Sunnyside at that time, the Hospital having operated on the site via inadequate, temporary wooden buildings from 1863. While only the West (Female) Wing of the Hospital was actually erected to his design (completed in 1876), its Gothic revival character influenced the design of the entire complex, which was finally completed in 1892 with the opening of the Administration Building.

This series consists of Staff Diaries and Communication Books from Queen Mary Hospital, Hanmer Springs.

The diaries record general notes, one page per day. The diaries are standard A4 sized page per day printed diaries.

Staff record appointments, telephone calls, meetings, pharmacy deliveries, reminders, volunteers and general daily notes.

The communication books are more haphazard, with staff sometimes recording meeting minutes and actions in detail, or simply brief general messages or reminders for fellow staff, as the need arose.

This volume contains handwritten and typed notes, lists, timetables and other tables which record the names of the student nurses, their course and exam results, courses studied, lecture timetables and other notes from 1960 to 1975. This record was presumably kept by the course administrator or head, and covers both junior and senior nurses.

Hand-drawn weekly timetables have been pasted or stapled in at intervals showing class times and subjects. Tables recording students' results in coursework and exams occur regularly throughout the volume. Student nurses' names, addresses and sometimes dates of birth are recorded at intervals - usually at the beginning of a term.
