	These individual patient files contain the papers relating to their admission, hospitalisation, treatment and death or discharge at Seacliff Mental Hospital.

	This series of patient files was started in 1912. The files were given a running number by date of admission and continued the numbering already used by the Statutory Papers (series 19850). Patients who were still in Seacliff Hospital in 1912 had their files moved from the Statutory Papers (series 19850) into this series.

	Most of the files in this series have been stamped on the front cover with the word 'Purged'.  In 1965 a circular was sent to all Medical Superintendents from the Head Office of the Department of Health Division of Mental Health.  The circular recommended that  the files of patient who were discharged between 10 and 50 years ago could be purged.  The papers to be removed included "leave forms, trivial correspondence, temperature charts, etc".  The following papers were to be retained - one copy of the preliminary statement, applications for admission and medical certificates, Clinical notes on forms M.H.57 and M.H.58, Medicine Cards, ECT Cards, Questionnaires and any correspondence likely to be of legal or clinical significance.

	(b) All Seacliff and Orokonui mental hospital patient casebooks are restricted for 100 years from the year of admission of the last patient in each casebook.

	(c) All public archives containing sensitive personal information about staff and students are restricted for 75 years from the date of the last paper or last entry in the record.

	(d) Individual patient archives will not be listed by name in the public finding aids of Archives New Zealand, nor will information confirming the existence of the public archive be made available, during the period the public archive is restricted.

	Access to restricted public archives and lists of restricted individual patient public archives, is not available without the written permission of the Southern District Health Board and any successor.
