A Department of Public Health was established in 1901 under the Public Health Act 1900.  The Act divided New Zealand into six health districts:  Wellington, Auckland, Dunedin, Napier, Christchurch and Nelson.  A district office was established in each health district.
Until 1920, district offices were administered by a District Health Officer who reported to a Chief Health Officer, who in turn reported to the Minister of Health.  The District Health Officers were given extensive powers and were medical practitioners with specialist knowledge of sanitary and bacteriological science.  In 1920 the role of District Health Officer was replaced with the position of Medical Officer of Health.
Over the ensuing years the number of district offices gradually increased:  There were 17 in 1960 and a maximum number of 18 were in operating in 1974.
